Health Department Temporary Food Service Application (TFSA) fee for any food (pre-packaged or prepared on-site) OR beverage (canned, bottled or poured): $55 if paid AND paperwork submitted* by April 3 / $125 if paid or paperwork submitted between April 4 through April 17 (final deadline). 

*On time payments with LATE paperwork submission will be charged the $70 late fee.
